In the US, the most polluted lake is Lake Onondaga in New York. In terms of the world, Lake Maracaibo in Venezuela has received scrutiny for being severely polluted.
According to a report by the Worldwatch Institute on nuclear waste by Washington D.C., Lake Karachay is the most polluted spot on Earth. Lake Karachay is a small lake in the Southern Ural Mountains in Russia. Beginning in 1951, the Soviet Union began using that lake as a dumping site for radioactive waste from a nearby nuclear waste storage and reprocessing facility. The lake accumulated 4.44 exabecqerels of radioactivity. For comparison, the Chernobyl disaster released between 5 to 12 EBq over a large area. If the load of radioactivity had leaked into the Arctic Ocean, it could travel halfway across the globe.
