It is hard to say which site is the “most” contaminated, as all contaminants are different and pose different risks. The EPA maintains a website that allows you to search among the sites considered National Priorities for cleanup, due to their high levels of toxicity (the site below). Information for the number of sites that have been successfully deleted (cleaned up) is also easily accessible from the site below.
California, for example, has sites that range from Asbestos mines to pesticide factories and Air Force bases.
Right now, probably in Japan. Nuclear contamination has been a problem in several areas around the world, unfortunately. People living and working around the Chernobyl accident in 1986 experienced extremely debilitating and often fatal cancerous diseases, and the groundwater was one source of the spread of radiation.
