Diseases impact plants and animals in incredibly diverse ways. Depending on the nature of the particular ecosystem, and the nature of the disease, the impact could be virtually nothing, to devastating.
No answer to this; far too many variables. No “worst” or “best” anyway, as those are entirely subjective.
I agree with rigibson. Ecosystems and biomes differ so much all over the world, and the life in all these places is so expansive and diverse that it is impossible to pinpoint a single disease or virus that poses the largest risk to nature as a whole.
