It might be useful if you would elaborate on what you mean by “form.” Knowing nothing more, my inclination would have been to say that the general form of a shopping cart is a rectangular prism made of interlaced steel bars set upon wheels. But it is hard to imagine that that is what you are after.
The form of a shopping cart is generally a boxed, metal container-carriage attached to a wheel base. They can also be found in plastic. There also can be found various sub-forms that have attachments at the front for younger children being brought along for the shopping trip.
