The most recently publicized example is the Gulf of Mexico, as many animals in that ecosystem have been killed or oiled by the BP spill. We still do not know how much oil is still in the ocean, nor do we know what the long term impacts will be. In addition to the oil released into the ocean, a large amount of dispersants was also used to mitigate the effect of the spill but we also do not know what kind of long term impacts that will have on marine life. For more information about how much oil was spilled and how many animals were affected, you can visit the site linked below.

Any place that has been subject to an open pit mine, or a clear cut lumber operation has been completely destroyed. Plants and animals may be able to recolonize these locations in the future, but they will never be the same.
