It’s been reported that had TEPCO vented their reactors and released heat sooner, the nuclear rods would not have melted so badly and the meltdown would not nearly have been so bad. The Tokyo government had told TEPCO to vent the reactors the day of March 11, but it wasn’t until the next morning that TEPCO decided to vent the reactors.
Supposedly, if the plant had been automated, that would have helped, in addition to what Phillius said.
