Is it unhealthy for a 15 foot maple tree if portions of the roots within 3 feet of the trunk are exposed?

The maple tree is quite large so having a portion of the root exposed is not unusual. If the tree is very close to a foundation or underground structure you will want to make sure the rest of the root is not pushing against the structure and potentially damaging the building.

The exposed portion of the root should be treated with care. Try not to run it over with the lawn mower so it because scraped up. If the inner part of the root becomes exposed, it may leave to disease or infection and cause problems for the tree. You may consider putting some mulch over the exposed root.
