The golden toad is definitely one. They were declining rapidly in the late 80s partly because of a fungus that grew on them called chytridiomycosis, or chytrid for short (pronounces kitrid). There are some others, especially birds, but it is difficult to really determine for sure if something has gone extinct because it would depend on how long it has been since someone last spotted it.
