It sounds like you have Pear Trellis Rust. This affliction is common in southern British Columbia and has been spotted in Ontario and Northumberland, UK. It can be treated with a copper spray that is purchased from garden centers in a wettable powder. Fall seems to be the recommended time. It appears that this is highly contagious, so all nearby pears, cedars, and junipers should be treated as well.
You can always call on someone from the agriculture department at your local college to help confirm the diagnosis.
