There is no definite number but the large majority uses human-based simulators rather than live animals. According to one website, over 90% use the simulators. I easily found a few instances of hospitals using live animals for training: one was at Mass General, where they used sheep for trauma training, and the other was at St. Louis Children’s Hospital, where they used cats. The military also has been known to use animals for military medical trauma training (http://www.humanesociety.org/issues/medical_training/).
