Moderately harmful. Using leaded gasoline puts more lead into the atmosphere, leading to widespread (but very small, in terms of the effect of one car) exposure. The ratio of lead to gasoline is low, but if millions of cars used leaded fuel, it would contribute to an increased number of lead-related birth defects among a population.
