    Amelogenesis (enamel formation has two main phases).  First, partially minearalized enamel is formed from by the enzyme alkaline phosphatase which acts on secreted proteins to form an enamel matrix. The second phase involves several proteins which form a more solid, striated matrix.  Both phases occur before the tooth errupts from the gum surface. 

    Evidently, we form our enamel abotu 4uM (thats 4 x 10^-6 meters) per day when our teeth are forming during the 3rd to 4th month of pregnancy.
