It doesn’t have to. Think about when you brush your hair, and you lose some. My family has had horses for a long time and we’ve used horsehair to make all kinds of decorations, jewelery, etc, and we get hair just by combing their manes and tails.
I suspect, though, that large scale suppliers of horsehair may not use the most humane practices. Looking at this website bowhair.com, it didn’t seem to me that there was any mention of the humane harvesting of horsehair, so I question it. You’d have to question them directly about their practices.
I would imagine that horse tails are either cut or carefully pulled out (probably not too painful, just like with people).
