Not an outright ban of ethanol but South Africa had a ban on the usage of maize for ethanol production.
Reports of Australia banning it citing illegal drag racing are available online. As far as the states, many states and cities have actually required that ethanol be in all the fuel sold in their respective state or city. Missouri, Minnesota and Hawaii have all passed legislation requiring that ethanol is blended with gasoline in motor fuels. Portland, Oregon passed legislation requiring that all service stations within the cities limits sell only gasoline that is at least a 10% ethanol blend.
