In the United States no major cities use untreated drinking water because people hold their governments to a very high level and would generally not accept untreated water (unless it came from a natural spring with naturally filtered water). In other countries however, a lot of people live without adequate water and are forced to drink water that is not safe for human digestion.
If it can be described as a city, it has a water treatment system. In the United States small towns sometimes rely on spring or well water, but it is filtered before it comes through the pipes and out of the faucet.
